Active Day

Active Day offers Adult Day Health Services in four distinct lines of business: Adult Daycare Services - Services designed to attend to the emotional, intellectual, physical and medical needs of the client based on the assessed level of care; Therapy Services - Services designed to increase mobility and functionality of the client through physical, occupational and speech therapy; Personal Care Assistance Services - Services in the home setting, including assistance with bathing, dressing, feeding and daily home living; & Transportation Services - Services designed to support the caregiver by providing non-emergency transportation to and from the facility or other medical appointments.

Communicare

Communicare is one of fourteen community Kentucky mental health centers providing mental heath, mental retardation and substance abuse services in the Lincoln Trail ADD District. This district includes the counties of Breckinridge, Grayson, Hardin, LaRue, Marion, Meade, Nelson and Washington.

Dreams With Wings

Dreams With Wings is a Louisville non-profit that provides quality community-based residential housing and support services to individuals with mental retardation, developmental disabilities and autism. It is their mission to empower these individuals as they recognize their strengths, contribute to their community and pursue their dreams.

Options For Individuals

The mission at Options for Individuals is to provide adult day training for individuals with mental retardation/developmental disabilities that:

1. Encourages each person to grow and develop according to his/her own capacity;

2. Fosters family stability;

3. Promotes meaningful community involvement and social growth;

4. Encourages each individual to offer his/her unique gifts and talents to the community.

Options Unlimited's mission is to assist people with disabilities to become as independent as possible by obtaining opportunities to participate in and contribute to their community through work, educational programs. and recreational programs. In order to achieve this mission, Options will operate an employment program, a day program, and social activities that will provide these opportunities.
